What Should I Expect?

All demographic, medical, and HIPPA forms for new patients must be completed prior to your visit. You can either download them from the links below or come 15 minutes early to your appointment to complete them in the office. Please remember to bring your photo ID, insurance cards, and a list of your current medications to each appointment. If this is your first visit, we will also need any significant ocular history records. If you wear contacts, please bring the box they came in or the most recent prescription you have for the contacts. Bring your most recent prescription glasses as well.

Minors under the age of 18, as well as patients who are not legally competent, must be accompanied by a legal guardian or responsible party at all times.

As a new patient, your first exam will be a comprehensive eye exam. Most visits will necessitate dilating your pupils, which will cause your vision to become blurry for a short period of time. You should bring sunglasses to wear home, as well as a driver if you feel unsafe driving with your eyes dilated.

Patients who do not speak English must bring an interpreter.
